Missouri Statewide Independent Living Council (MOSILC) IL Grant Application FY13
In order to simplify the grant application the information is now divided into two documents:
- IL Grant Application Instructions and Forms. (Maintain at center for reference.)
- IL Grant Application (Application documents need to be completed and returned to VR.)
The completed application is due back to DVR by May 13, 2013.
Please plan accordingly so that your board of directors has an opportunity to approve the grant prior to the May 13, 2013 due date. For centers with board meetings late in the month this might require having the grant ready for approval at the April board meeting or convening a special board meeting to obtain grant approval. The grant application is to be submitted electronically as an email attachment using Microsoft word to Tammy McSorley.
Please base your FY 2014 grant budget on the FY 2013 IL grant amount received since FY2014 state budget information is not available at this time. We will notify you of any changes in the FY 2014 award amounts in the grant award letter.

As a reminder, the grant application summary and the assurances pages must be signed by the appropriate official and a hard copy mailed to DVR for our files. An original signature, not a typed or printed name is required on these pages. Mail to:
